Job Title: New Arrivals Intake & Case Manager

Summary of Job Duties:

Helps clients to navigate safety net government services for individuals and families by providing case management, information and referral services, and interpretation and translation services. The four program goals are 1) to increase enrollment of eligible immigrant and refugee individuals and families in government safety net programs; 2) ensure that these families and other limited English-speaking persons are able to thrive; 3) assist them to become self-sufficient, fully integrated members of their communities; and 4) increase awareness and understanding of availability and eligibility for health and human service programs within the immigrant community.

S/he works under and reports to the New Arrivals Project Manager. This is a part-time position of 23 hours per week that requires flexible hours worked off-site.

 

Specific Responsibilities:

  • Provides case management services, including assessment of client needs, the development of individual case plans, one-to-one interactions, assistance in applying for services, and follow-up as needed
  • Delivers information and referral services
  • Assists in interpretation and translation services
  • Conducts community presentations and workshops,
  • Maintains accurate records and prepares required reports on these activities
  • Attends required staff meetings
  • Performs other duties as assigned

 

Qualifications:

  • Bilingual (Spanish/English)
  • High school diploma/GED; college degree preferred (A.A. or B.A.)
  • One year’s experience in and an in-depth understanding of the Latino immigrant and refugee community and its issues
  • Knowledge of human service programs, eligibility criteria, processes, and regulations
  • Willingness to attend training on and learn about new developments for public health programs
  • Computer proficiency (Word and Excel)
