Job Title: Illinois Family Support Program (IFSP) Intake & Case Manager
Summary of Job Duties:
The Illinois Family Support Program (IFSP) Case Manager provides critical support to families by guiding them through government services. Helps clients to navigate safety net government services for individuals and families by providing case management, information and referral services, and interpretation and translation services. This role focuses on helping individuals and families access safety net programs, promoting self-sufficiency, and fostering integration into the community. Responsibilities include case management, advocacy, and providing interpretation services.
S/he works under and reports to the Public Benefits Department Director. This is a full-time position of 40 hours per week that requires flexible hours worked off-site.
Specific Responsibilities:
- Conduct comprehensive client assessments to determine needs and develop tailored case management plans.
- Guide clients through the application process for government programs and ensure follow-up.
- Assist in interpretation and translation services as needed.
- Maintain accurate client records and prepare required activities reports.
- Attend staff meetings and trainings as required.
- Perform other duties as assigned.
Qualifications:
- Bilingual in Spanish and English.
- High school diploma or GED required; Associate’s or Bachelor’s degree preferred.
- One year of experience working with the Latino immigrant and refugee community.
- Strong knowledge of government health and human service programs.
- Proficient in Microsoft Word and Excel.
